  • Not billed alongside G0568, G0569, 99492, or 99493 for the same patient and month

Who can bill

  • Physicians and non-physician practitioners directing clinical staff, who are also billing APCM for the same patient

Eligibility & billing requirements

  • Patient is enrolled in APCM for the same month
  • Patient has a behavioral health condition being treated by the billing practitioner
  • Same 20-minute clinical staff time, care plan revision, and monitoring requirements as 99484
  • An APCM code billed for the same patient and month

When to use it

General BHI for a patient already enrolled in APCM

Clinical staff spend 20 minutes on monthly depression monitoring and care plan updates for a patient already billed under APCM, the practice bills the APCM-specific BHI add-on instead of the standalone 99484.
