Eligibility & billing requirements
- Active RPM episode with a qualifying connected device in place
- 2 to 15 days of physiologic data automatically collected and transmitted in the 30-day period
When to use it
Partial-month data collection
A patient's device only transmitted data for 10 days during the billing period due to travel, falling short of the 16-day threshold for 99454.

- Device data log showing 2-15 days of readings in the 30-day period
Common billing mistakes
- Billing 99454 instead of 99445 when fewer than 16 days of data were actually collected
